Otherwise is an adversative conjunction or a noun (The animal was not a dog, otherwise a wolf). But, meanwhile, is the conjunction "Yes" followed by negation adverb "not" (But you eat the food, there will be no dessert).
The sequences "but" and "if not" are homophones, that is, they sound phonetically the same, but their meaning is different and they are written differently. That is why in orality, when hearing any of these forms, confusion can be generated.
