A proof of out-of-hospital delivery It is a requirement requested in some places as proof for procedures, such as to carry out the corresponding record of the children or taken for the labor process of the incapacity period of the mother.
This document can be made by a community authority (in the case of a rural community), by the municipal president, by a private doctor, or by an authorized midwife.
In case the midwife is not authorized or it is a natural delivery without assistance, it is a doctor who is responsible for presenting the certificate or to the Public Ministry which is responsible for drawing up a certificate of evidence and ultimately to a notary public who she could attest.

I also declare that I am in good health.
