Example of a Work Permit Letter

The work permit letter is addressed to a hierarchically superior person, in employment terms, and that is empowered to grant permits for non-attendance at work or rendering of services extraordinary.
It can be used both in private companies, or for state companies, as well as in government institutions.
They can be addressed to an official in charge or to a specific person; this depending on the degree to which the writer of the letter is.

May I request from you a work permit of a month and a half that is essential for health reasons.
Due to an attempted assault and physical assault on the street, I have been injured and received seven stitches in one leg and four in one arm.
These injuries were classified by the authority as second degree, and will keep me inactive for a month or month and a half.
I am sending you a certificate from the doctor who is treating me who certifies the need for rest and the urgency of this permission.
