The undersigned Eduardo contreras Ramírez, with address at calle álamos number 22 Col Pátzcuaro Ecatepec Estado de México, I appear before you to present the following:

I hereby appear before you in order to exercise the appeal for reversal, in order to extend the deadline to present the documentation of medical history that is given to me solicitous.

This is due to the fact that for reasons beyond my control, the IMSS will deliver these documents to us until the 26th of this month.

FACTS:

On November 8 of this year I suffered a traffic accident with which I stayed in the hospital for a week, and because my Family members were not present and the person who helped me did not have my work information, this could not be reported circumstance.

As a result of these injuries, I was unconscious for six days, having been induced into a coma by medical order, this in order for my body to react and in all this time my workplace could not be reported.

Due to the foregoing and because said document was required from me within a period of no more than three days, I present this document with the purpose of increasing said period due to the fact that said documents are delivered to me by the medical institution that attended.

I sincerely and respectfully ask:

FIRST.- Have me in time and form presenting REPLACEMENT RESOURCE.

SECOND.- Kindly revoke your agreement dated December 12th of this present “in which you order the presentation of documentaries consisting of medical reasons and medical expenses regime.

THIRD.- Please dictate a new agreement to extend the term to present the documentaries that have been requested, for after December 26, since for reasons beyond my control, I cannot obtain them before.

I protest what is necessary.

I hereby request a replacement period for the presentation of my final work in English, due to events of a medical nature that made me totally unable to do so.

Facts

Last Monday we had to enter a relative for emergencies, which caused a consumption of time which left me outside the deadlines set for the presentation of my work.

Although it did not affect me physically, it did consume my time because I gave priority to the most important thing that is health and life.

I have already submitted my corresponding excuses along with a justification letter and the documents that prove the event, now I request a school make-up in relation to work or an extraordinary exam to comply with the matter.

Waiting for your prompt and favorable response, I say goodbye.
