Here are some sample letters requesting safety equipment. This document must be presented to the area in charge of industrial safety within the company. As it is a security team, it must be delivered in a timely manner, so it is essential to request it from the person in charge, being able be the personnel manager or the personnel security department, clearly depending on the company and type of work done.

The undersigned by Luis Adolfo Tomillo Guzmán, hereby certify the lack of adequate working conditions due to not having the necessary items and marked as "E.P.I" in the company affecting the employees under my charge, who require their "E.P.I" which is the individual protection equipment which must be delivered directly to the workers, items that consist of on:
- Crash helmet
- Protection gloves
- Protection mask
- Industrial boot
- Polypropylene suit
- Fall protection harnesses
- Safety or protection goals at work and
- Protection gloves.
- Risk management manuals
These protective supplies "e.p.i" must be applicable to the 598 employees who work in the plant as laborers and also to the 63 security officers, including myself, who have the right to have all security services applicable.
Waiting for the delivery of said protective supplies, it will be necessary to suspend all the work that may generate extensive risk because the items requested are very necessary for your realization.

This is due to the fact that said supplies have not been sent and they are essential to carry out the work with the security levels established by the security regulations.
It is essential that these supplies be sent as soon as possible, since the level of insecurity has delayed the work and it is necessary to catch up on the work.
